Employee Share a Concern Form

This form is designed to provide Beacon City School District employees with an easy method to report specific information related to an issue or concern.

You are not required to complete the entire form for it to be submitted. However, if the report does not contain some specific information, an effective response is difficult. 

Responses for both the print and online forms are directed to John Giametta, Executive Director of Human Resources and Operations, for initial review.
