Sports Physical Information

    If your child is going to play a school sport, they must have two things on file with the school nurse: Health Appraisal (Physical Exam) and Health History. The student can not try-out or participate in a sport until they have been cleared by the school health practitioner.

    Students with a Current Health Exam (done within the last year) on file in the nurse’s office:

    • Health History must be completed by the parent/guardian on Family ID (see the link below). It must be updated prior to the start of every season in which they plan on playing.


    Students who need a Health Appraisal (Physical Exam):

      • 1 ~ Student can get a physical from their private doctor and a copy of the NYS School Health Exam Form (see link on bottom of page) must be submitted to the school nurse. The student can bring it to the nurse's office, it can be uploaded onto Family ID, faxed or e-mailed directly to the school nurse. 

      • 2 This option is suspended as this time due to Covid -19.  Students must get physical from private physician:~ Student can get a physical at school from the school physician, free of charge. A Physical Consent must be completed on Family ID. School physicals are scheduled on a first come first serve basis on designated days during which the school physician is working in the district.

    • Health History must be completed by the parent/guardian on FAMILY ID (see the link below). It must be updated prior to the start of every season in which they plan on playing. 

     

    The BCSD is offering FREE sports physicals for all interested students in grades 7-12.  A sports physical, along with Family ID registration is required for all Beacon athletes.

    Physical appointments will be offered on a first-come first serve basis.  As a reminder, athletes are not authorized to participate in sports unless a valid physical is on file and the athlete is registered and approved on Family ID.
