Sports Physical Information

    If your child is going to play a school sport, they must have two things on file with the school nurse: Health Appraisal (Physical Exam) and Health History. The student can not try-out or participate in a sport until they have been cleared by the school health practitioner.

    Students with a Current Health Exam (done within the last year) on file in the nurse’s office:

    • Health History must be completed by the parent/guardian on Family ID (see the link below). It must be updated prior to the start of every season in which they plan on playing.


    Students who need a Health Appraisal (Physical Exam):

      • 1 ~ Student can get a physical from their private doctor, a copy of which must be submitted to the school nurse. The student can bring it to the nurse's office, it can be uploaded onto Family ID, faxed or e-mailed directly to the school nurse. 

      • 2 ~ Student can get a physical at school from the school physician, free of charge. A Physical Consent must be completed on Family ID. School physicals are scheduled on a first come first serve basis on designated days during which the school physician is working in the district.

    • Health History must be completed by the parent/guardian on FAMILY ID (see the link below). It must be updated prior to the start of every season in which they plan on playing.
